How does one tell the difference on the D200 between the USA model and the import model ?

You will know when you have problems in the first 2-3 years and you can't get if fixed by NikonUSA for free. You will also notice the manual isn't in English... if you get one. You may have to pay extra to get what comes normally with a USA version.

Try looking at the bottom if the camera offten you can tell if it is from around hear by if the writing is in Japanese or another language that is not English. Also, I'm not sure, but if you know, or can find the number code on the camera you may be able to track to see if the code is a US model or one from over seas. I hope this will be of help to you.

The real thing is this one: there's no virtually difference in both... BUT as everyone knows, it's not the same to buy something from Taiwan, Japan, or Germany. Why's this? Simple. It just depends on the quality controls it recieves before it's done. In Taiwan, it's cheaper to make the product... but controls are very soft. In Japan, the controls are more heavy... but from Germany (where's actually most expensive to make something), you just know there is no possible failure on the product.
If that's not what you was asking for... then look at the box, and it will surely say that.
