Nikon D90 non-std flash, fires once, then no flash until pulled and reconnected. How do I fix?

I am taking photos of the eye through a Topcon Photo slit lamp biomicroscope. The D90 is mounted to a beam splitter using a T-mount adaptor. The Topcon flash connects through a PC cord to a Nikon hot shoe adapter. I have a problem:

FLASH DISABLES AFTER ONE SHOT - The camera fires the shutter and flash. When a second shot is attempted there is shutter release but no flash. If I disconnect the PC cord and reconnect it, the flash fires once again upon shutter release. Can we make it fire for successive shots without the PC disconnect-reconnect?

Sounds like a driver issue. Could also be dirty hot shoe contacts or even that the third party lens isn't recommended for use on the settings you're using. Have you tried full manual? If it syncs there, it may be your only choice with that flash.
